Get ready for school in one visit, back-to-school vaccinations and physical exams at the CHC’s will be available. Services are available for children ages 4 years - 18 years old. Private insurance, Medicaid, and MIP accepted. Sliding Fee discounts available for uninsured and underinsured patients. Children must be accompanied by an adult.

What to bring:

• Child’s immunization record
• Child’s birth certificate
• Parent/Guardian ID

Guardians must provide proof of guardianship or parental authorization
